Money Follows the Person
A federal Medicaid demonstration that helps eligible nursing-facility residents return to community living by funding transition services and shifting long-term services funding to home and community settings.
Money Follows the Person exists for the resident who is in a nursing facility but could live in the community with the right support. It funds the transition and shifts long-term-services funding back to a home or community setting. Availability varies, so the nursing-facility social worker or the local health plan is the place to start.
