  1. Inspections. What state inspectors found on site and in complaint investigations. This is the hardest rating to game and the closest thing to a direct look at care.
  2. Staffing. How much nursing time residents get. The list shows the reported nurse hours per resident per day, so you can compare homes directly.
  3. Quality measures. Resident outcomes such as falls, pressure injuries and rehospitalizations, reported by the home.

Then look at the flags: an abuse citation, a place in the CMS Special Focus program for persistent problems, or federal fines in the last three years. Any of these is a reason to open the full inspection report, linked under each home, before you go further.

  2. If my parent comes in on Medicare for rehab and then needs to stay, can they keep the same bed once Medi-Cal takes over?
  3. What were the findings on your last state inspection, and what changed afterwards?
  4. How many residents does one nurse aide care for on day, evening and night shifts?
  5. How long have your director of nursing and administrator been here?
  6. Can we visit at different times of day, including a weekend?

When a nursing home is not the answer

A nursing home is for people who need nursing care around the clock. Many families arrive here after a hospital stay and later find that assisted living, memory care, or care at home fits better once rehab is done. If your parent may qualify for Medi-Cal, the Assisted Living Waiver and IHSS can pay for care outside a nursing home.

How are the star ratings worked out?

CMS gives every certified nursing home one overall rating and three separate ratings: health inspections, staffing, and quality measures. The inspection rating comes from on-site state surveys and complaint investigations, the staffing rating from the nurse hours the home reports, and the quality rating from resident outcomes. Will Medicare pay for a nursing home?

Medicare covers a limited stay of skilled nursing or rehab, generally after a qualifying hospital stay and only while daily skilled care is needed. It does not pay for long-term custodial care. For a long stay, families pay privately, use long-term care insurance, or qualify for Medi-Cal.
