Every facility below has agreed to accept Assisted Living Waiver payment, which is what “takes Medi-Cal” means for assisted living in California. The list comes from the Department of Health Care Services’ own open dataset, verified 2026-08-29. It is a record of participation, not a ranking, and nothing here is a recommendation.
Read this before you call
- Participation is not availability. Every facility here has agreed to accept Assisted Living Waiver payment. None of them is promising an open bed. Capacity moves week to week and the only way to know is to ask.
- Waiver participation is not memory-care approval. A facility that takes the waiver still needs separate CDSS secured-perimeter approval before it can legally admit a resident who wanders. Ask for that record specifically, on the first call.
- The waiver does not cover room and board. It covers the care services. The resident contributes their income toward rent, so ask each facility what its room-and-board charge actually is.
- You need an approved waiver slot first. A facility cannot admit you on the waiver until DHCS and a Care Coordination Agency have approved eligibility and a slot is available.

How to read this list for San Diego
The licensed-capacity figure on each listing is the most useful number here and the one nobody explains. In San Diego the average is 21 beds, with 20 six-bed residential homes and 3 facilities of 50 beds or more.
A network weighted toward small homes changes the search itself. Instead of touring a few large communities you are calling many houses, most with no website and no marketing staff, where you will often reach the owner directly and where one opening decides everything. Budget more calls and fewer tours.
Where San Diego sits in San Diego County
Families searching in San Diego usually have more geographic room than they assume, and the waiver does not care which city you land in.
