Los Angeles · Medi-Cal assisted living

Assisted living that accepts Medi-Cal in Los Angeles, California.

48 assisted-living facilities in Los Angeles, California accept Medi-Cal through the Assisted Living Waiver, holding 2,929 licensed beds between them, an average of 61 beds per facility. California Care Compass compiled this list from the Department of Health Care Services' open dataset, verified 2026-08-29. Participation means a facility accepts waiver payment; it does not mean a bed is open.

The quick answer

How many
48 participating facilities in Los Angeles, 2,929 licensed beds, averaging 61 per facility.
What kind
19 are six-bed residential homes and 22 have 50 beds or more. They are different products: a house with one or two caregivers on shift, versus a campus with tiered care levels.
What the list proves
That each facility accepts Assisted Living Waiver payment. Not that a bed is open, and not that it can admit a resident who wanders, which needs separate CDSS approval.
Before you call
You need full-scope Medi-Cal with no share of cost and an assessment at nursing-facility level of care. No facility here can settle either for you.

The 48 facilities in Los Angeles

Every facility below has agreed to accept Assisted Living Waiver payment, which is what “takes Medi-Cal” means for assisted living in California. The list comes from the Department of Health Care Services’ own open dataset, verified 2026-08-29. It is a record of participation, not a ranking, and nothing here is a recommendation.

Read this before you call

  • Participation is not availability. Every facility here has agreed to accept Assisted Living Waiver payment. None of them is promising an open bed. Capacity moves week to week and the only way to know is to ask.
  • Waiver participation is not memory-care approval. A facility that takes the waiver still needs separate CDSS secured-perimeter approval before it can legally admit a resident who wanders. Ask for that record specifically, on the first call.
  • The waiver does not cover room and board. It covers the care services. The resident contributes their income toward rent, so ask each facility what its room-and-board charge actually is.
  • You need an approved waiver slot first. A facility cannot admit you on the waiver until DHCS and a Care Coordination Agency have approved eligibility and a slot is available.
  • Records are from the DHCS open dataset, verified 2026-08-29. If a listing is wrong, the authoritative version is the DHCS list itself, linked in the sources below.

How to read this list for Los Angeles

The licensed-capacity figure on each listing is the most useful number here and the one nobody explains. In Los Angeles the average is 61 beds, with 19 six-bed residential homes and 22 facilities of 50 beds or more.

A network weighted toward larger communities means fewer calls and more tours, but waiver beds are usually a small share of a large community's census, so participation overstates practical availability. Ask what share of residents are on the waiver today.

Where Los Angeles sits in Los Angeles County

Families searching in Los Angeles usually have more geographic room than they assume, and the waiver does not care which city you land in. The full county-by-county list shows what else is within reach, which matters because a facility with an open bed twenty minutes away beats a waitlist around the corner.

Common questions

4 entries

Which assisted living facilities in Los Angeles accept Medi-Cal?

48 facilities in Los Angeles participate in California's Assisted Living Waiver, which is the only way Medi-Cal pays for assisted living in this state. Each one is listed on this page with its licence number, address, phone number and licensed capacity, taken from the DHCS open dataset and verified 2026-08-29.

How much does Medi-Cal cover at these Los Angeles facilities?

The waiver pays the care-services portion of the bill. It does not pay room and board, which the resident covers from their own income, usually Social Security. Ask each facility what its monthly room-and-board charge is, because it varies and it is the number that decides affordability.

Do any of these Los Angeles facilities take memory-care residents?

Some will, but waiver participation and memory-care approval are two separate records. A facility needs CDSS secured-perimeter approval before it can legally admit a resident who wanders or exit-seeks. Ask for that specifically on the first call rather than at the tour.

What if nothing in Los Angeles has an opening?

Look at the rest of Los Angeles County first, then at adjacent counties. In the meantime IHSS can pay for care at home, including by a family member, in every California county. If care needs are at nursing-facility level, Medi-Cal skilled nursing is available statewide without this waitlist.
